With the exception of Aetna, SFHC participates
in a large number of health plans-- however, we cannot keep track
of each persons' individual insurance requirements for pre-authorizations,
covered procedures, etc. Please bring
your insurance plan book to your appointment with you for easy reference
in the event you may require quick
access to the list of rules regulating the scope of your insurance
coverage.
If we are not directly contracted with your particular insurance plan,
we are happy to bill it for you as a courtesy.
If you have a 'secondary' insurance plan, we will provide you with
whatever information we can to assist you in
billing your carrier.
Payment of your 'Co Pay' as determined by your insurance plan is expected
at the time of your visit.
We have a very reasonable payment scheme available to those of you
who either choose not to use your
insurance or who are uninsured. Please call or fax us for more details.
Patients who are paying on a cash basis or 'out of plan" receive
automatic enrollment in the
SFHC ultimate Access Plan™– click the link below for details.
We accept Visa, MasterCard, American Express and Discover.
The Ultimate Access Plan is a program dedicated
to the creation and maintaining of a "Personal Physician"
relationship-- a collaborative planning and treatment to achieve your
"personal best".

Medicare
We will file all paperwork for our Medicare patients. We do not accept
assignment for office visits.
We ask that you pay your portion at the time of your office visit
and via our billing process, we will request
that Medicare reimburse you directly.
